Nothing much or nothing new and then return with how about you?

A: "What's new?"
B: "Nothing much, I got my exam results yesterday, I passed!"
What's new is asking what you've done recently.

What's up means "What's happening in your life?"
A: "What's up?"
B: "Nothing/Not Much/Just the usual. You?"
Or,
B: "All kinds of stuff, I went to lunch with my friends yesterday. On Wednesday I saw my family. Thursday I saw a movie..."
Usually just answering 'Not much' is fine with these questions as by saying that it indicates that you're fine. :)

@Lillian_Yuri: Yeah it's hard to write yes to them, it doesn't make sense. I don't like answering those types of questions, saying 'nothing' seems a bit rude.
"How are you?" is much better.

You can say, "not much, thank you/ thanks" this usually lets people know that you're okay.

I normally see "what's up?" as a more informal "how are you?" I normally get a "what's up?" if someone thinks I'm upset so I will reply normally with "nothing, you?" I know some people to reply with what they're doing.
But "what's new?" I see it as someone is asking you if you have done anything recently, I normally reply with if something has happened in my life, like I passed an exam, I crashed my car etc. Again you can just say nothing much, you?

Sometimes you can answer "what's up?" by saying "what's up?" back to the person. It might not be necessary to actually answer the question. It depends on whether the other person is expecting an answer or is just using it as a greeting. If they continue talking about a different topic without giving you a chance to answer, then that is what's happening.

@TheBlip: yes when my friends use it as greeting, they don't answer and ask them same! I think when face to face it's okay :)
